hi. my former employer told me that i have to write my "recommendation letter" myself.
could somebody take a look on my text if it´s correct?

I am writing to confirm the former employment of [name]. [Name] had been employed with [company name] from [date] to [date]. Her positions, dates and nature of activities were as follows:
Position 1: Journalist; associate editor-in-chief
Dates: From 2001 (June) - to 2001 (December)
Duties: Writing articles for the computer magazine CDtip, speech correction, collaboration with the graphic artists, supporting the editor-in-chief
Position 2: Journalist
Dates: From 2002 (June) – 2005 (December)
Duties: Contributing to the monthly special supplement on PCs for the daily XY
During our collaboration I valued
- her willingness to work more than required
- independent fullfillment of the tasks
- her creativity, and
- her willigness to to improve the work of the whole department.

also ich würde das auch eher ausformulieren, macht immer nen besseren Eindruck würde ich sagen 
z.B. so:
I am writing to confirm the former employment of [name]. [Name] had been employed with [company name] from [date] to [date]. She worked as a Journalist from June till December of 2001, writing aricles for the computer magazine CDtip, including speech correction, collaboration with the graphic artists and supporting the editor-in-chief. In her second Period for our company, which happened from June 2002 till December 2005, she contributed to the monthly special supplement on PCs for the daily XY.
I rarely noticed such a high motivated, skilled and creative employee as [name] and can just advice [name] to nearly every over company interested in good Journalists.
Ich würde das ganze außerdem noch in Briefform bringen, für was ich leider im moment keine Zeit mehr habe! Ich hoffe ich konnte dir helfen
