I guess "sie" below must refer to "Ursache," since that is the only feminine noun I detect -- so the sentence seems to be saying that "the cause is in addition a symptom... "  Correct?

And the reason the possibility of a mistake with "sie" occurs to you, I guess, is that "ist darüber hinaus" seems like it should refer to Primat, true?

Not because it sounds like it should. It's just that Primat is the only other word that could be referenced in any sensible way.
