I have a question regarding grammar, hopefully you guys would help me out.
I have noticed many times that third form of verb comes in simple present sentence, sure it comes in present perfect, for example in sentence like these:
- date of birth written form
- last seen in November
- power given to junior officer
Could you explain which composition of grammar is this...would be appreciated

It's difficult to tell based on the examples, a full sentence would have been better.
In all of your examples the "verb" is a participle. Participles are derived from a verb to modify a noun phrase, similar to an adjective. They are sometimes called verbal adjectives for that reason.
However, the reason why that participle is used differes between your examples. In the first example the participle is just an adjective while in the other two examples, the participle is part of a reduced passive relative clause.
